Corrective Action Plans for PerformanceMeasure: Chronic Condition and Disease
Management (Q)
1 Are treatment plans developed and documented in the medical record by a provider within thirty (30)
days of identification that the inmate has a CC? [P-G-01, CC 2.20.2.4]
Level 1 User: Troy Evans Date: 8/22/2013 1:28:53 PM
Corrective Plan: Reviewed all listed charts. All charts listing does not have peak flows do have peaks flows. It is on
the Chronic Condition form in the vital section at the end. The other charts where labs were missing, labs have
been scheduled. The process at Safford for CC labs is to schedule labs 2 weeks prior to there CC appointment so
the provider has them in hand at time of appointment. Inmate cc not due until end of September and labs have
been scheduled. Inmate number could be incorrect, shows up in aims as inactive since 2001. Inmate scheduled
8/28/2013 for Gabapentin level.
